How to get your own medical pocket card
Here is how you
can get your personalized medical pocket card.
STEP 1:
Determine Medical Information
The
process begins with the information that we receive from you for
your medical pocket card. All individuals must be of legal age
- 18 years or older - and taking medication on a daily basis. The
medication submitted should not change very often and should remain
consistent for 1-2 months or more. For example, Diabetes,
Heart Condition, Epilepsy, Alzheimer and Cancer medications.
STEP 2:
Select Design & Order Information (Part I)
We will guide you through our comprehensive online
Design Gallery and Order Form to choose a special picture that you would like on
your personalized medical pocket card. After choosing your
design, you will be taken through a secured payment process.
STEP 3:
Complete Order Information Form (Part II)
Once
your card has been purchased, you will be directed a secure order
form to complete your order. Please fill in all your pertinent
medical information as requested on the form. (You may also print our
secure online form, complete it at home, and mail, fax or email
it to us).
STEP 4:
Verification
Now that we are at the completion of the Order Form process, all
information must be accurate and will be verified. Once
verification is confirmed from our secured online data form, then
all your pertinent information is formatted to your personalized
medical pocket card.
STEP 5:
Delivery / Shipping
The sooner we get your information, the sooner you will benefit from
using your medical pocketcard. Please allow 2 weeks for
delivery. Remember, all shipping is FREE!
